Short courses in Global Health at the Witten/ Herdecke University

Content overview

• Introduction, the evolution of global child health (GCH), main actors and global initiatives, burden of disease, epidemiological trends and goals
• Key concepts of global child health (e.g. life course approach, social and environmental determinants of child health, developmental origins of health and disease, Convention of the Rights of the Child and child health advocacy)
• Major contributors to child morbidity and mortality across the life course (most, but not all of the examples used in the sessions come from settings with limited resources)
• Clinical and public interventions to improve child health and well-being (e.g. promotion of exclusive breastfeeding, nurturing care, immunisations, oral rehydration solution and zinc, use of triage systems and emergency care (e.g. ETAT), Integrated management of childhood illness (IMCI)

Learning objectives

At the end of the course participants should be able to

• Analyse a child health related clinical or public health problem and apply main concepts of Global Child Health
• Describe communicable and non-communicable diseases that are major contributors to global morbidity and mortality in new-borns, children and adolescents
• Explain key clinical interventions and public health strategies to address these conditions

Target group

Health professionals with at least a Bachelor’s degree (e.g. in nursing, midwifery, medicine, public health), health managers, social scientists, tropEd students

Teaching and assessment

A mix of interactive lectures and seminars, and more student- driven methods such as supervised group work and self-directed group work guided by specific question form the basis of most sessions. Lectures and seminars draw on essential and optional prior reading. In addition, brief presentations of group work results, role-play, a clinic simulation and a debate will promote active learning.

Assessment of learning is by a mini-Essay (60%) and a single best answer MCQ (40% of the final mark) at the end of the course.

Prerequisites

The course is held in English-excellent command of the language is a requirement.

Content overview

In week one, after the module introduction, we develop the theoretical foundations of the roles and responsibilities of npHCWs in low resource settings:

• The module focuses on the following cadres of npHCWs: nurses, midwifes, clinical officers and other types of non-physician clinicians, community health workers and volunteers.
• Basic statistics of npHCWs globally and regionally (cadres, geographical distribution, density, demographics including gender, qualifications, professional development and training, places of work, salary/remuneration,).
• Interprofessionalism: definition and underlying concepts
• npHCWs, primary health care and UHC: current global and selected national guidance and policies (including their role in task-shifting and task sharing, health promotion and prevention activities)
• Differences and similarities in approaches to teaching, research and advocacy between high and low- and middle-income settings

In week two, we underpin the theory with practical examples from the literature and participants’ and facilitators’ field experience. Students will apply their theoretical knowledge to their practice. Examples may include:

• Challenges and opportunities of the introduction of a specialist Clinical officer cadre in Sierra Leone
• Nursing care during the Ebola response in DR Congo: enablers and barriers
• Volunteers’ and Community health workers’ roles in the HIV response: experience from Malawi
• How work Medical Doctors and npHCW together in hospitals in Nigeria?

Time is dedicated during week two for students to develop their posters and presentations using their self-selected setting and cadre. If needed, we will offer technical guidance on poster development.

Learning objectives

At the end of the module, the student should be able to:
1. Analyse roles and responsibilities of non-physician health care workers (npHCW) in the context of Universal Health Coverage.
2. Demonstrate a profound knowledge of npHCWs development opportunities for leadership, research and advocacy
3. Discuss service delivery models of npHCW in primary health care to ensure interprofessionalism and quality of care in low resource settings

Target group

Students who have completed a core course at a tropEd home institution are considered fluent in the English language as well as students who have completed at least a Bachelor course taught in the English language are considered fluent. Other students need to show results of 550 in the English TOEFL test or 213 computer-based or 79/80 internet-based or IELTS band 6.0 (not older than two years).
Students have to have a professional background in health (e.g. medicine, nursing, midwifery).

90 hours: 35 hours contact time (25 hours lectures and seminars, 10 hours supervised group work), 55 hours self-directed (20 hours guided group work preparation, 35 hours individual reading and assessment preparation).

Description: At the end of the module, the student should be able to:
1. Explain the basic concept of equity and equality in child health.
2. Demonstrate knowledge about child rights and entitlements and how they can be violated.
3. Analyze the context, trends, root causes, and contributing factors to the inequity and inequality in child health and child healthcare.
4. Discuss ways to ensure that children receive their rights and entitlements and draw on the role of families, communities, governments, and children themselves.
5. Criticise policy approaches that promote inequity and inequality and deny child rights.

Assessment Procedures: Both assessments are in class and on the last day of the module. There will be a group presentation after the mini-essay test. Depending on the number of participants, groups will be formed (3-5 participants). To familiarize students with the assessment format, we conduct a mock examination for each assessment tool at the end of the first week, which is peer-assessed using the assessment rubrics. The course coordinators will clarify any questions that arise.
1. Mini Essay (60%): The Mini Essay will be 60 minutes. Students can choose between two essay topics where they need to analyze an inequity and inequality child health scenario and apply the main concepts discussed during the course that may help to offer a solution.
2. Group presentation (40%): Presentation followed by discussion (Q&A). Students can choose the country of the presentation and are expected to cover the following: background, existing child and child health inequities in the country, health system, existing government policies and strategies, and critically analyse the policies promoting child rights.

Content:
1. Module Introduction: General module structure, learning outcomes, and assessment information.
2. Key concepts I: Introduction to the "Convention on the Rights of the Child" concept, root causes (including social and cultural norms) of inequity and inequality in child rights (and in health), social determinants of child health; right to health care services (acceptability, accessibility, affordability, availability).
3. Key concept II: Measures/assessments of child health inequality and child rights in general; child health inequalities and their consequences to the society and slower development.
4. Key concepts III: Health system and policy strategy to address inequity and inequality in child rights and in health (e.g., gender-responsive health system, closing gender data gap)

Prerequisites: Students from outside the tropEd network must provide evidence of a professional background (e.g., a Bachelor degree) in health (e.g., medicine, nursing, midwifery, dental) but do not require any previous knowledge in child health or child right.

Over five days, students of all health professions will be introduced to clinical and public health related issues of global children's health in an interactive form during a virtual trip to health facilities in rural Malawi.

Learning goals:

• You acquire an overview of frequently encountered clinical pictures.
• You will get to know important concepts of global child health that will help you to classify these clinical pictures.
• They develop a feel for non-technical skills necessary for successful on-site practice. Teaching methods: lectures and seminars, group exercises, casuistry, role-plays

Learning materials and a detailed schedule will be provided on Moodle one week before the course starts. The teaching languages are German and English.

The course ends with an exam (ungraded) and the handing over of the participation certificates.

WHO Pocket Book

In 2018 Farzana Yasmin and Ralf Weigel became members of the Editorial group for the WHO Pocket Book of Primary Health Care for Children and Adolescents. The WHO launched the  Pocket Book on 21st March 2022. In addition, Ralf Weigel acted as editor and author for a series of articles published in a BMC Pediatrics supplement that provided evidence for Pocket Book’s chapters on preventive interventions.  

Child Health Accountability Tracking (CHAT) group

Since November 2018,  Ralf Weigel works as a member of a technical advisory group with WHO and UNICEF. The Child Health Accountability Tracking group  (CHAT) has the following objectives:

• To provide a platform for sharing and discussing the work of key child health and well-being initiatives and relevant work of UNICEF and WHO aimed at the achievement of child health related global goals and targets.
• To identify priority activities and measurement gaps in child health and well-being.
• To create a catalogue of standard indicators and validated tools for measuring and monitoring child health and well-being indicators.
